2017 Pre-Seminar Forms are Available!
Attention all 2017 HOBY Ambassadors!
We are very excited to have over 200 student ambassadors attending the HOBY TGC seminar this year! Before you can attend the seminar, there are some Pre-Seminar Materials/Forms that need to be filled out by May 19, 2017. Please work with your parents to make sure that these forms are returned on time. Closer to the seminar, you'll also receive a packet of "last minute instructions" for the weekend.
HOBY uses an online system for our pre-seminar forms. These will require inputs from both you (the student ambassador) and a parent/guardian. It's very important to fill these out accurately, including dietary restrictions that you have.
The Pre-Seminar Forms can be filled out online HERE.
In addition to the online forms, there are also three forms that will need to be printed off and returned:
Risk Form - Must be notarized by a Notary Public and returned via standard postal mail.
Service Project Volunteer Waiver - Must be signed and returned (either email or postal mail with the Risk Form) to participate in the Saturday Service Project
Medication Form - If you will be bringing any medication to the seminar, this must be signed by your doctor and returned (either email or postal mail with the Risk Form)
