Pre-Seminar Materials for the HOBY TGC 2019 Leadership Seminar will be sent out in March!

All currently registered students + parents will have received an email with all details. (Please add "hobytgcrecruiting@gmail.com" to your contact list to ensure that our emails don't end up in your spam folder!)

There are THREE forms total that need to be completed

​

1. Online forms (via formstack) can be completed here: https://hoby.formstack.com/forms/hobytgc_2018_pre_seminar_materials 
    

2. Consent & Acknowledgment of Risk_Form – NOTARIZE.  Scan/email a copy to hobytgcrecruiting@gmail.com and BRING WITH YOU TO THE SEMINAR.
    

3. Medication Verification Form for Physicians – Only required if you are taking prescription medication at the seminar.  Must be signed by prescribing Physician and scanned/emailed or bring it with you to the seminar.

If we do not have both the Online Forms AND the Risk Form, you will not be allowed to stay at the seminar!  As of 6/5/19, we are still missing the Risk Form from approximately 20% of students.  PLEASE complete this form.  Contact us if you are unsure whether your paperwork is complete or have any questions.
